Privacy Notice and Cookie Statement

J.Bok Dental Practice respects your privacy. We make every effort to safeguard your privacy and we will ensure the information you provide to us is treated confidentially. We process personal data in accordance with the applicable data privacy legislation and regulations. This Privacy Notice and Cookie Statement explains how we handle your data.

Categories of personal data
When you use our website and the services offered on the site, you provide certain data to us. The same applies when we carry out the treatment contract in our practice. Some of those data may be personal data. We only store and use the personal data that are provided directly by you or that are clearly intended to be processed by us at the time they are provided to us.
The service used by you determines what data we collect. This includes the following:
Name, address and place of residence
Gender
Email address
Telephone number
Data regarding your health
Name of your healthcare insurer
Name of your other healthcare providers
Date and time of your appointment
Payment details

Legal basis for data processing
We can only legitimately process your personal data if we have a legal basis on which to do so. Your personal data are processed by us because this is necessary to perform the agreement between us, as determined in Article 6(1)(b) of the General Data Protection Regulation (GDPR). We may also process your personal data in connection with other legitimate interests, such as informing our patients regarding new developments or changes to our services. This legal basis is outlined in Article 6(1)(f) of the GDPR.

Purposes of data processing
The personal data collected by us are used for the purposes below:
Creating your account so we can register you with our practice
Logging into your account
Keeping your medical records up to date
Scheduling appointments
Carrying out treatments
Remembering your preferences
Sending newsletters
Improving our services
Delivering other services as requested by you.

Disclosing your personal data to third parties
In principle, we will only disclose your personal data to third parties if you have provided your consent for this. We may disclose data without your consent if this is necessary to perform the agreement that exists between us or if we have a legal obligation to do so.

Retention periods
We will only keep your data for as long as is necessary to fulfil the purposes set out in this Privacy and Cookie Statement, unless the law requires otherwise. Your medical records will be kept for the statutory retention period of 15 years, as established in the Dutch Medical Treatment Contracts Act (Wet op de geneeskundige behandelovereenkomst, WGBO). We will also apply retention periods based on legislation other than the Medical Treatment Contracts Act as required.

Security measures
We have put in place appropriate security measures to protect your data as effectively as possible.  Our practice complies with the NEN7510 standard for information security management and has developed and implemented a privacy policy.

For your convenience, we have included links to third-party websites on our website. Please note that if you visit such websites, the terms and conditions outlined in the privacy statements of the relevant third parties will apply. Use of cookies
We use cookies in order to operate our practice website. Cookies are small data files that may be automatically placed on or read from a visitor’s device, such as their desktop, tablet or smartphone. This is done via the web browser of the device. There are different types of cookies. These are the types of cookies we use:
Analytics cookies – these allow us to analyse how visitors use our website and then use the information gathered to make improvements;
Our website places cookies by US-based company Google, as part of their Analytics services. We use these services to track and generate reports on how visitors use our website. These services are not used in the case of logged-in users. We did not grant Google permission to use the Analytics data obtained for other Google services and IP addresses are anonymised. The information collected by Google is transferred to and stored on servers located in the United States. We have entered into a data processing agreement with Google. The data collected by Google will be anonymised as much as possible. We have no control over how Google and/or third parties use the data. Google may provide the data to third parties if the law requires this or in order to allow third parties to process the data on Google’s behalf. Google abides by and has signed up to the EU-US Privacy Shield developed by the US Department of Commerce and the European Commission. Please refer to the Google Privacy Statement for more information on this data processing of that type.

Your rights
You have a number of rights you can exercise in relation to personal data you have provided to us. These include the right to access, correct and delete your data. You also have to right to ask us to provide you with a copy of your data, to transfer your data to another party and to restrict the processing of your data. You can also object to your data being processed in certain ways. In addition, you can withdraw your consent for the data processing at any time. Changes to this Privacy Notice and Cookie Statement
We reserve the right to amend this Privacy Notice and Cookie Statement. We will publish any changes on our practice website. We recommend that you check this notice and statement regularly to keep up to date with any changes.
